Shift: Monday-Friday 8: 30AM-4: 30PM

Position is direct hire with the company. Candidate will be eligible for full benefit package after 90 days.



Possess any of the following educational requirements: Doctor of medicine or osteopathy, Occupational therapist or Occupational therapist assistant, Physical therapist or physical therapist assistant, Psychologist, Social worker, Speech-language pathologist or audiologist, Professional recreation staff person, Professional dietician, Human service professional including human behavior, human skill development, or a Bachelor level degree where there is sufficient coursework (at least 30 semester hours) in human services area.

Other Qualifications: Must also get 16 CEUs (training in your field) per year, have a clean, valid driver s license, verified background check, required to participate and pass the DSP training course, must be in good health and demonstrate emotional stability, willingness to work beyond normal hours, and in other positions in the event of call-offs, must attend all in-service training and staff meetings, assist in training new personnel regarding policies and procedures, job description and duties.

Job Summary: Case management for vocational clients, including conducting and implementation of individual service plans and behavior plans. Supervise and direct staff, oversee all aspects of client care, and activities. Assure health and safety standards are met. Planning, supervision, and follow up of daily work and social activities.

Immediate Supervisor: Assistant Director, Director



Essential Functions include the following duties and responsibilities, broken into categories:

QIDP: the company will help you obtain this certification if selected
  • Responsible for individual service plans and behavior plans
  • Responsible for overall supervision of all direct support staff
  • Oversee all aspects of client care, assuring that medical needs are directed to the day training nurse and other management
  • Assure that the overall needs of the individuals at day training center are being met
  • Responsible for staff schedules and floor coverage
  • Takes call-ins and assist with coverage
  • Schedule and assist with in-service training
  • Assure programs are being implemented
  • Assure documentation on programs is being done daily
  • Review programs on an on-going basis
  • Assure that monthly data is taken, calculated and documented in a timely manner
  • Attend ISP meetings and follow up on ISSA recommendations
  • Schedule all community activities
  • Assure that data is taken on community outings, along with pictures of outings
  • Quality Assurance checklist done weekly; sent to Assistant Director
  • Member of various committees, along with preparation for the meeting: Safety committee, Human Rights, Behavior Management
  • Convene and assure documentation on monthly Client Council Meetings; assure that actions are followed up
  • Oversee menus and food purchase orders
  • Responsible for ordering cleaning supplies, with purchase order
  • Oversee grocery shopping and assure that all food items are provided for meals
  • Responsible for accounting accountability, such as checks and accurate timesheets
  • Communicate with all residential sites, including clients family homes
  • Promote teamwork by assisting co-workers in the care of clients, regardless of staff assignments
  • Responsible for new client admission: orientation to facility, explain rights/ document this was done, set up new chart, complete face page for chart and for Director. ICAP, SLOF, Psycho-social history, complete satisfaction survey interview and other assessments as necessary
  • Schedule 30-day, 6-month, and annual ISPs
  • Complete ISP for ISSA review
  • Send a copy of the ISP to the individual, guardian, ISSA, residential facility and family members
  • Complete monthly QIDP notes by the middle of the next month
  • Change program goals according to start and target date
  • Assist clients with seeking and maintaining employment outside of day training center
  • Responsible for periodic client training, such as clients rights and sex education
  • Assist dining area during meal time to ensure clients programming and medical/dietary needs are being followed and monitored
  • Maintain a clean and safe environment through performance of daily cleaning
  • Other duties as assigned by supervisor



New Employees:
  • Assists in scheduling interviews
  • Be a part of the interviews
  • Assists with calling potential employee references prior to hiring
  • Schedule new employee orientation with Director and Assistant Director
  • Assist with training of new staff/ with training checklist
  • Schedule all new employees for physical exam at the beginning of their employment
  • Complete new employee file with each staff, including I-9 and State Police background check (make a copy of employee file for home and send original to Director for employee chart)
  • Assure that each employee receives an initial TB skin test, along with an annual TB test
  • Assure all new DSPs are trained within 90 days
  • Review employee shift duties as duties change and revise as needed
  • Review, complete, and turn in time sheets to the Assistant Director the last day of each pay period
  • Assist with employee evaluations



Health and Safety:
  • Contact maintenance person when needed as soon as possible
  • Follow cleaning schedule and assure facility is clean and safe
  • Assess, along with day training nurse, medical, health, and safety of the individuals attending day training
  • Assure that med counts are done timely and correctly
  • Responsible for monthly fire drills, assure there is follow up resolution per drill
  • Responsible for disaster kit inventory and maintenance
  • Responsible for first aid inventory and maintenance
  • Assure that fire extinguishers are inspected annually
  • Assure that fire suppression system is inspected quarterly
  • Review all incident/accident reports, communicate to day training nurse, and report to Safety Committee quarterly
  • Bring significant changes, behaviors, illnesses, etc. To other QIDPs and Assistant Director



Client Charts/Finances:
  • Keep all client charts filed in a timely manner
  • Responsible for keeping charts up to date for review by ISSAs and state inspectors
  • Responsible for maintaining clients records and charts
  • Responsible for client s money that is kept at day training; keep a record of receivables and expenditures



QIDP Assistant Duties:
  • Prepare for client s ISPs by getting staff s input
  • Complete new program recommendations
  • Responsible for new program implementation per ISP recommendations
  • Responsible to put all program documentation sheets in documentation binders by the 1st of each month
  • Responsible for follow up of program implementation
  • Responsible for vocational assessments
  • Quality Assurance checklist done weekly; sent to Assistant Director
  • Ensure that clients are appropriately attired for the weather and that clothes are kept clean and presentable for work
  • Assist dining area during meal time to ensure clients programming and medical/dietary needs are being followed and monitored
  • Maintain a clean and safe environment through performance of daily cleaning
  • Convene and assure documentation on monthly Client Council Meetings; assure that actions are followed up
  • Monthly attendance sheets completed and sent to the Director by the 1st of each month
  • Communicate with all residential sites, including clients family homes
  • Other duties as assigned by supervisor

Physical Expectations:
  • Must be able to transfer clients if necessary, using gait belt or hoyer lift
  • Must be able to assist clients to restroom with moderate transferring from wheelchair to toilet
  • Must be able to perform moderate cleaning tasks (mopping, sweeping, vacuuming, laundry, etc.)
